Disclosing Mistreatment using MIMS

The purpose of this web form is to provide a confidential disclosure mechanism for learners in the Faculty of Medicine to identify incidents of mistreatment they have personally experienced or witnessed in the learning environment. Learners include individuals registered in the Doctor of Medicine (M.D.) program, a residency program or a Faculty of Medicine graduate program.

This web form is NOT FOR EMERGENCIES. Emergent incidents should be reported to Campus Enforcement and Patrol at 709-864-4100 (St. John's), 709-637-2888 (Grenfell) or to 911. Below, please find a list of emergency resources you can utilize for support.

This web form is NOT FOR REPORTING SEXUAL HARASSMENT OR SEXUAL ASSAULT. Anyone may anonymously seek the advice of the Sexual Harassment Advisor.

Please note that this system is not monitored on a 24-7 basis. All forms will be reviewed within 10 working days.
