Staff, faculty and students of Memorial University who test positive for COVID-19 and were on-campus within the 48 hours prior to first symptom(s), or if asymptomatic the 48 hours prior to the test being performed, are required to complete this form. This form will allow the university to assist with contact tracing and notify individuals considered close contacts that were in attendance at activities required as part of learning, teaching or working. Close contacts are individuals with whom interactions occurred without masks.

For non-university organized activities such as having lunch with friends or colleagues on campus, attending events such as mixers or other events organized by third-parties, the individual is responsible for notification.

For students requiring mental health supports contact Student Wellness and Counselling Centre. Employees feeling stress for any reason are encouraged to avail of resources such as the Employee Assistance Program (EAP) at no cost, 24 hours a day, seven days a week. Reach out to supervisors/professors directly to discuss sick leave or absences.
